Premium Plan – A plan offers a broad network with 100% coverage for diagnostic and preventive services (as defined in the plan design), and 50% for all other covered services with a 12 months waiting period.  The plan includes an annual $2,000 plan maximum and $100 lifetime deductible/person.  This plan requires you keep the coverage for 12 months unless you qualify for a mid-term termination.  

Ascent Plan – offers an ample benefit backed by the large network with a shared maximum for families. 100% coverage for diagnostic and preventive services (as defined in the plan design), 80% coverage for sealants, and 50% coverage for fillings, dentures, root canals, crowns. No waiting period. An affordable plan that rewards members for maintaining dental coverage by reducing the patients cost sharing over a three-year period Starts at $1,000 annual and can go up to a $1,500 annual maximum in year three. This plan requires you keep the coverage for 12 months unless you qualify for a mid-term termination.  

Enhanced Plan – Offers an ample benefit backed by the large network.100% coverage for diagnostic and preventive services (as defined in the plan design), 80% coverage for sealants, and 50% coverage for fillings, dentures, root canals, crowns, and more (12-month waiting period may apply). The plan includes an annual $1,000 plan maximum. This plan requires you keep the coverage for 12 months unless you qualify for a mid-term termination

Clear Plan – This plan tells you in advance how much you’ll pay for dental work, and that’s exactly what you pay – nothing more! With the Clear Plan, you pay a fixed dollar amount for services like an exam (including a checkup, cleaning, and bitewing X-rays), filling, crown, root canal, etc. There is no deductible, no annual maximum and no waiting periods for coverage to begin.

Basic Plan – The most affordable option covers preventive care, fillings and non-surgical extractions.

Individual Exclusive Provider Organization (EPO) Dental through Willamette Dental You’ll work with an exclusive provider organization who can combine the best available scientific evidence with clinical experience to develop a personalized oral health plan for you.
